Simply download the below forms by using the blue form title and then edit them for your businesses use. Contact me if you have any questions about the below processes. Steps 1 - 4 is the set up. Steps 4 - 8 is ongoing.

1. Add your Logo, company name and emails to the QR Code form. You can add as many emails you wish but be aware that individuals will need to manually add the emails to, and send the form. Contact me to automate this process if needed.
2. Print out the QR Code form and place it on site. (Specifically on your scaffolding).
3. Amend the toolbox talk to suit and deliver to all site personnel.
4. After the individuals scan the Scaffold Hazard Form QR Code Notice, fill it in (Point and Select only) and add the emails, the Scaffold Hazard Report Form will be emailed out.
5. Management uses the link in the Scaffold Hazard Report Form to open and create an Action Plan.
6. This Action Plan Form is emailed out to a single or multiple people, or to the scaffold business to fix the Hazard.
7. After the Hazard has been fixed the action person completes section 2 of the Action Plan Form and emails back to the person who sent it.
8. Complete the Monitoring Worksheet so you can track this Safety Initiative and report the findings back to the site.

All forms are retained from the emails and need to be stored in your own system.
